Sticky Header and Booking does not work

The sticky header does not work.  The booking section does not work. I cant see the pick-up and drop-off locations in the dropdown.

Okay, thank you. I have checked it already. The pick-up and Drop-off locations mean I have to fill them in the back-end? Was this section meant to work with Google Maps?
